Visitor Release Form
1. Acknowledgment of Risks
Visitor acknowledges that training dogs and visiting Trainer’s property involves inherent risks, including but not limited to:
Injury to Visitor and/or persons accompanying Visitor to Trainer’s premises, such as bites, scratches, or other physical harm.
Injury to Visitor’s dog, including but not limited to bites, scratches, or other physical harm.
Risks associated with water, including drowning or waterborne illnesses.
Risks associated with firearms, including accidental discharge or hearing damage.
Risks associated with uneven ground, including trips, falls, or other injuries. Other potential hazards on the property, including but not limited to exposure to noxious odors, sanitation issues, noise, and increased traffic.
2. Assumption of Risk
Visitor knowingly and voluntarily assumes all risks associated with training activities, including, but not limited to, those listed above.
Visitor understands that these risks can result in serious injury or death to themselves or their dog.
3. Waiver of Liability
In consideration of being permitted to visit Trainer’s premises and participate in training and other activities, Visitor hereby releases, waives, discharges, and agrees to hold harmless Trainer, Trainer’s agents, Trainer’s employees, and Trainer’s representatives from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by Visitor, or to any property belonging to Visitor, regardless of the cause, including but not limited to the fault or negligence of Trainer or Trainer’s agents, employees, or contractors.
4. Indemnification
In consideration of being permitted to visit Trainer’s premises and participate in training and other activities, Visitor also agrees to indemnify, defend, and hold harmless Trainer, Trainer’s agents, employees, and contractors from any and all claims, damages, demands, penalties, costs, liabilities, losses, and expenses (including reasonable attorneys; fees and expenses) arising out of or relating to any act, omission, negligence, or willful misconduct of Visitor or Visitors dog.
5. Compliance with Laws
Visitor agrees to comply with all applicable laws and regulations, including but not limited to those related to the control and management of dangerous dogs as defined by North Carolina law.
6. Miscellaneous
This Agreement constitutes the entire agreement between the parties and supersedes all prior agreements or understandings, whether written or oral, relating to the subject matter hereof. This Agreement may only be amended or modified by a written agreement signed by both parties.
